Has anyone else experienced medical coding errors. I am in the midst of trying to correct an error made by a coder that resulted in an false diagnosis.
The medication I take for fatigue requires 6mo visits to the dr. After a May 2017 visit, I noticed a document in my records that indicated an outpatient mental health visit and a diagnostic code for ADD. I am still trying to have my records purged of this document. All communications with the dr's clinic have been verbal and I cannot get a written confirmation of the error.
I have been told by phone that the error was based on an assumption of the coder as to the medication involved and vague dr notes. The Dr NEVER made the diagnosis.....it was the coder.
I live in a small town and have limited access to health care and now feel cut off from my dr.
I was shocked at how non chalant the clinic and hospital system have been to this error. It is bad enough having an MS diagnosis.....I do not need additional diagnosis that are false.
I have been purusing this since the beginning of June when I caught the error and still have not had resolve.
I filed a complaint with dept of health and human services and have not heard anything.
Does anybody have any ideas?
* It is now September and I am still trying to get this fixed. I have been locked out of my online records by the clinic/hospital, my insurance(s) still have not been billed with the corrected code and I am being told that the document I was trying to have removed has been removed but I cannot verify it.
I have filed complaints with the state medical licensing and with HHS and have heard nothing.
I do take medications that are running out....yet I have no confidence in the clinic system and accurate record keeping.
I dont know what else to do. This is very frustrating. I feel completely cut off from medical care at this time.
Hi Frizzyroo, I have not heard of this type of error before! When individuals want to file a complaint against a doctor/hospital/clinic we refer to the State medical Board, I know that you can file a variety of complaints through them but am unsure if this includes medical coders.
